What is CPHL?
CPHL is a Managed Care Plan that serves Medicare, Medicaid Managed Long Term Care, and Medicare/Medicaid Dual Eligibles in New York City, with over 60,000 members residing in Brooklyn, Bronx, Manhattan, Queens, as well as Staten Island (Richmond), Nassau and Westchester, Rockland, Erie and Niagara counties.
CPHL has delivered high-quality health care in New York City’s communities for more than 15 years. Their goal is to provide eligible members with access to quality healthcare so that they can continue to live healthy and productive lives in the least restrictive manner possible.
CPHL offers the following products:
- Medicaid Advantage Plus (MAP) covers all Medicare-covered and Medicaid-covered services.
- Medicare Advantage Part D (MAPD) covers all Medicare-covered and prescription drugs (Medicare parts A and B).
- Institutional Special-Needs Plan (I-SNP) covers Medicare members who require institutionalized care.
- Dual-Eligible Special Needs Plan (D-SNP) covers members who are dual-eligible for Medicare and Medicaid, with a full comprehensive plan.
- Managed Long-Term Care Plan (MLTC) covers members who are covered for Medicaid with long-term care services.
Note: HealthCare Partners (HCP) is contracted with CPHL for all lines of business except MLTC.
