VillageCareMAX is a Managed Care Plan that serves Medicaid and Medicare dual eligibles in New York City, with over 24,000 members residing in Brooklyn, Bronx, Manhattan, Queens, as well as Staten Island (Richmond), Nassau and Westchester counties.*

VillageCareMAX offers the following products:
- VillageCareMAX Medicare Total Advantage Plan (HMO DSNP) (MAP) covers all Medicare-covered and Medicaid-covered services. These include all Medicare Part A & B services, Part D prescription drugs, and all Medicaid-covered services, including community-based long-term care.
- VillageCareMAX Medicare Health Advantage Plan (HMO DSNP) covers Medicare-covered services and includes all Medicare Part A & B services, as well as Part D prescription drugs.
- VillageCareMAX Medicare Health Advantage FLEX Plan (HMO DSNP) covers Medicare-covered services and includes all Medicare Part A & B services, as well as Part D prescription drugs. Includes a FLEX benefit to provide even richer dental, vision and hearing supplemental benefits.
- VillageCareMAX Medicare Select Advantage Plan (HMO) – NEW IN 2024 is a Low-Income Subsidy plan (LIS) intended for members eligible for Medicare and qualify for Extra Help. This includes Part D prescription drug coverage along with supplemental benefits such as vision, hearing, and dental.
- VillageCareMAX Managed Long-Term Care Plan (MLTC): A Medicaid Managed Long-Term Care Plan (MLTC) that provides members with long-term care services and support like personal care, adult day health care and others. This product is for Medicaid-eligible individuals.
