At HCP, we are committed to maintaining the highest standards in healthcare. To meet Centers for Medicare & Medicaid Services (CMS) requirements, ensure claim accuracy, receive timely reimbursements, and provide our members with the most current information, it is essential that all care providers contracted with HCP keep their demographic information up to date.
How to Submit a Demographic Change
There are two ways to update your demographic information:
1. Submit demographic changes directly, using doForms. Enter your NPI# and TIN# into the appropriate fields, then select your name from the “Confirm Provider Name” field. Your demographic information will populate. You may then confirm or edit the fields as needed. Once completed, click “Submit” at the top right of the page.
2. Fill out the Demographic Change Form. For Billing name, address, or TIN modifications/additions, please include the attached W-9 form with the most up-to-date information. Please complete and email the form to database@hcpipa.com or fax to (516) 746-8473.
Please be sure to verify all information, including:
- PCP office hours: To qualify as a PCP, New York State requires that a provider practice a minimum of two days per week (16 hours) at each primary care site. For example, if a provider has three office locations and practices, 16 hours per week at locations #1 and #2, but only 8 hours per week at location #3, he/she could be qualified as a PCP only at the first two sites, but not the third. This information is specific to the provider, thus we need to know the hours the provider is in the office, not the hours that the office is open.
- Office Email Address: The email address of the primary provider office contacts.
- Electronic Medical Record (EMR) Type: Name of the EMR used by your office(s).
