Ensuring your Patients Remain Covered
Some health plans require annual renewal to avoid loss of coverage. Encourage your patients to renew their plans to support continued access to preventive health services, reduce emergency room (ER) usage, and support the overall health of our community.

- EmblemHealth Enhanced Care (Medicaid)
- Enhanced Care Plus (Health & Recovery Plan, aka HARP)
- Child Health Plus (CHPlus)
- Essential Plan (EP)
How to Ensure your Patients Retain their Access to Care
- Encourage members to update their address and contact information with their agency (and opt-in for text messages, if available), to ensure their renewal information will reach them. This may include:
- New York (NY) State of Health
- Local Department of Social Services (DSS)
- New York City Human Resources Administration (HRA)
- Make members aware of the renewal process and how it works, for example:
- Renewals occur monthly and begin with those who have coverage through 6/30/2023.
- Deadlines will be based on a member’s coverage end date.
- Based on their end date, members will receive an email, letter, or packet from their agency once it is time to renew their health insurance. This notice will inform them of how to renew and the time allotted to respond.
- Inform members when coverage should be renewed and how to find their end date:
- Coverage should be renewed 90 days prior to the member’s end date (recertification date).
- To find their end date, members can:
- Contact their agency
- Sign-in to their EmblemHealth member portal or
- Call the HealthCare Partners Customer Engagement Team at (800) 877-7587 (TTY: 711), Monday through Friday, 8:30 am to 5:30 pm.
- Provide members with appropriate contact information and resources, e.g.:
